New Clients: Your first visit to the Healing Gardens Health Center

Thank you for your interest in becoming a patient at the Healing Gardens Health Center. If you are ready to take a deep dive into your health, you are in the right place!

Deb Lieberman, PA-C, and Bry Kring, NP, are currently accepting new patients, though Dr Jacqueline Fields is not; all the providers go over every patient together and collaborate on solutions and the wellbeing of each of our patients.

Please print, fill out, and return all the attached paperwork to our office with a narrative describing your health journey, what you are looking for from our office and what you’ve tried.  Please scan and email, hand deliver, or mail your paperwork back to us for review.  We are unable to print photos of paperwork.  We will then get new patient requests in front of the providers and see where we might fit you in. Although this process may seem lengthy or tedious, we pride ourselves in the amount of time we spend with every patient at every appointment. 

 

Our office is in network with UMR, United Healthcare, and Cigna Open Access Plus and PPO until June 1, 2024. 

As of June 1, 2024, we will no longer be contracted with insurance companies other than Cigna. And thus, will no longer submit your bills to insurance on our end. We will continue to work with Cigna for the time being, but will be exiting all insurance including Cigna by year end. We will keep you informed and certainly give you 90 days notice prior to exiting Cigna.

 

What this means for you: We are no longer contracted with insurance companies. However, you may still submit your superbills to your insurance on your own for reimbursement. The new process is that payment is due at time of service and we will provide you the documents to submit to your insurance company for reimbursement. We can provide you with a Superbill with the appropriate codes for you to submit to your insurance if you are out of network, please be mindful that we are unaware of what each plans out of network reimbursement schedule is.  New patient intake exams are $375- $425 for an hour-long appointment, and follow-up appointments range from $240-$340 dependent on how long the appointment is.

We can also order your lab work, procedures, and tests through an in-network provider and utilize a hybrid of care to best meet everyone’s needs.

We have researched and considered this change very carefully and ultimately found the additional burden is small compared to the freedom and significantly increased benefits that this change affords both of us. To help you navigate this change, we will work with you, explaining the superbill and what information your insurance company will be looking for.

Become a patient with us

The Healing Gardens is now accepting new patients on a case-to-case basis. If you’re interested in seeing one of the practitioners at the Healing Gardens please follow the steps below:

  1. Download & fill out the New Patient Intake forms below. Submit the intake forms to our office.

  2. Contact the clinic by phone to let them know you’ve submitted forms. The number is: 970-472-6789.

  3. Send in your most recent or pertinent health records to our fax number: 970-472-6799

  4. You will be contacted once we’ve assigned a practitioner to your case.

Your First Visit

Your first visit will last 60-90 minutes and includes a comprehensive interview process, along with a physical examination. Treatment plans will usually include education on lifestyle and diet to address your medical concerns.

You should plan to obtain any copies of your old medical records and we ask that you forward those documents to our office prior to your visit. You will be required to fill out your New Patient Packet before we can schedule your appointment. Our staff will review these forms prior to meeting with you in order to maximize your visit time.
