Patient Forms

Thank you for taking the time to print and complete these forms prior to your first appointment. Please print, fill out and return the below paperwork.

Scan and email them to healinggardenshealthcenter@hotmail.com with a narrative describing your health journey, what you are looking for from our office, and specific wellness concerns or goals you’d like to accomplish. Please scan and email, hand deliver, or mail your paperwork back to us for review.  We are unable to print photos of paperwork.  We will then get new patient requests in front of the providers and see where we might fit you in. Although this process may seem lengthy or tedious, we pride ourselves in the amount of time we spend with every patient at every appointment.
